  1. All documents after MBBS (Photocopy)
  2. NMC Registration Certificate - MBBS, Cardiology Speciality (Photocopy)
  3. Citizenship Photocopy
  4. Passport Size photo - 2
  5. Membership Fee - 5000/- (It is to be paid after approval in EC meeting)
